Before PT Java Volcano Rendezvous was formalized in 2023, the same operation ran at the same Bondowoso address as Ijen Bondowoso Homestay. Founder Mr. Sam (Bripka Agung Sambuko) operated the guesthouse from 2015 and is named in the Stefan Loose Reiseführer Indonesien as the operator at the homestay address.

Audit disclosure — legacy OSS record. A second NIB — 0220001393513 — may appear in legacy OSS portal records linked to JVTO. If you encounter it during verification, treat it as a possible legacy registration; the canonical active licence is 1102230032918 (shown in the credentials table above). Disclosed proactively for transparency: the legacy record has no impact on current operational status — TDUP, HPWKI, BBKSDA, ISIC, and INDECON registrations all reference the canonical NIB.

JVTO's legal credentials extend into operational health compliance. Under BBKSDA regulation SE.1658/KSA.9/2024, every visitor ascending Kawah Ijen must present a health certificate from a licensed clinic. JVTO coordinates this directly, end to end, with verifiable government-licensed parties:

• Regulation issuer — BBKSDA Jawa Timur. Balai Besar Konservasi Sumber Daya Alam, the Ministry of Environment body managing the Ijen Crater nature reserve. It issued SE.1658/KSA.9/2024, the supporting authority for the mandatory pre-climb health certificate.
• JVTO coordination — Klinik Bakti Husada, Bondowoso. A Ministry-of-Health-licensed clinic. Screening is conducted at the guest's hotel before the guide briefing and checks SpO₂ and blood pressure before ascent. The certificate is issued as a BSrE (Balai Sertifikasi Elektronik / BSSN) electronically-signed document — a legally valid e-signature under UU ITE, traceable to the doctor's SIP.
• Licensed physician — Dr. Ahmad Irwandanu. SIP (Surat Izin Praktik) and KKI (Konsil Kedokteran Indonesia) registered; STR QN00001073380217 valid to 2031-01-06, independently verifiable via the national health registry at satusehat.kemkes.go.id.
